FotoMend Order/Quote Form

Price ranges listed are the average for that category and are quotes only. The final price may be higher or lower than what is listed depending on the condition of the item.

Please print this Order Form, fill it out and do one of the following:

  • Contact FotoMend via phone or email to discuss your order.
  • Scan your order and email a copy to fotomend@hotmail.com.
  • Send your order to us along with your photos/slides/negatives/documents in the mail.

FotoMend may contact you to clarify specific points on your order or to make suggestions.

After placing an order and sending your photos to FotoMend, you will receive an email with a copy of the sales order including the work order number and estimated due date.

Upon completion of the project, FotoMend will send a Paypal invoice or a regular invoice (if paying by check or money order) to your email address. After your payment is received and cleared, an email with a copy of the paid invoice and the shipping tracking number will be emailed back to you.
